Photo 3.5*4.5 cm: Recent photo sized 3.5 by 4.5 cm.
Medical examination and psychological test: If required for your type of driver’s license.

*Important for Ukrainians: To exchange a Ukrainian driver's license, you must have a certificate stating that the person is not subject to military service, or a contract with a Polish company for a driver position + legal stay in Poland for more than 180 days (Residence card, visa, or PESEL UKR).
